For professionals - how to access our service
By referring your patients and their families to the Midhurst Macmillan Service you will be ensuring that they have access to specialist, compassionate palliative care. It will be tailored to their needs, enabling the patient to remain in their own home, or less often and where appropriate a local care home.
Patients registered with the General Practitioners from the following list of Practices can be referred to the service:

Liphook & Liss Surgery
Riverside Kelsey Surgery
The Grange Surgery
Haslemere Health Centre
Riverbank Medical Centre
Referrals will be accepted from any healthcare professional provided the patient, carer and General Practitioner are aware and in agreement with the referral to the service. It is important that the patient or carer is aware of the services available to them.
Referrals can be made for any patient over the age of 18 experiencing complex problems arising from a cancer or other life limiting illness.
Referrals can be made according to the following criteria:
- Those with severe, intractable complex symptoms.
- Those (and their carers) having difficulties in adjusting to or coping with their disease - psychologically, spiritually or emotionally.
- Where information and explanation is required relating to the illness, treatment, care options and allied support services.
- Relatives experiencing difficulties in bereavement, who would benefit from specialist support /further psychological intervention.
- To assess the need for further specialist unit services, i.e. day care or inpatient care.
- Healthcare professionals who require specialist advice and support with case management.
Discharge/Open Access Guidelines
All patients and carers known to the service will be made aware that they may be discharged or placed on open access (not receiving regular review but can be re-activated if their needs change). They may be re-referred again by any health care professional, patient or carer according to the above guidance, if their needs or circumstances change at any stage in the future.

Letter If you are sending in the referral form by letter please send it to Midhurst Macmillan Service, Midhurst Community Hospital, Dodsley Lane, Midhurst, West Sussex, GU29 9AW. If possible please use a copy of the electronic referral form, however, a full referral letter sent by email is also acceptable.
Relevant medical and social information should be sent with the referral including recent test results and treatment plans. Referrals will be triaged to the most appropriate team member who will contact the patient or carer.
Access to the Service
The Service operates 7 days a week from 8.30am to 8.30pm.
Response times Telephone contact will be made with the most appropriate person within 48 hours depending on the urgency of the referral. Appropriate interventions will be arranged from that point.
Out of hours support All patients, carers and healthcare professionals will have a contact number for their local Out of Hours (OOH) support.
Where admission may be required Wherever possible patient and carer choice will be adhered to. If care at home is the place of choice, provision of specialist and generalist care will be provided wherever possible utilising the statutory NHS services and the specialist palliative care team.
